Congratulations to Dr. Hidehiro Iwakawa, Dr. Prashanthan Sanders, and the full author team on their new publication in Heart Rhythm Society: "Leadless pacemaker, right ventricular pacing burden, and heart failure." Guidelines have long used 20% and 40% right ventricular pacing (RVP) thresholds for transvenous pacemakers, but it hasn't been clear whether they apply to leadless pacemakers. Using real-world remote monitoring data from PaceMate® (3,167 leadless pacemaker patients across 71 centers in 31 states and more than 45,000 transmissions), the team found: Baseline RVP of 20% or higher, and of 40% or higher, identified patients at increased risk of a heart failure diagnosis. Heart failure risk rose steadily as pacing burden increased, about 3.5% for every 10% increase in RVP, with no single safe cutoff. 38% of patients saw their pacing burden change by 10% or more over follow-up, which shows the value of tracking it continuously rather than only at implant. Current leadless systems don't have established upgrade pathways, so these findings point to the importance of careful patient selection and close follow-up for patients expected to need a high degree of ventricular pacing.
