Post-workout soreness is common. But for physical therapists, the more useful question isn't simply, “Is the patient sore?” It's: What does the patient's overall response tell us about the exercise dose? Delayed-onset muscle soreness (DOMS) commonly occurs after strenuous or unfamiliar exercise and can be particularly pronounced following novel eccentric exercise. But soreness alone doesn't tell us whether a workout was effective or whether someone is ready to progress. A few things matter clinically: Consider the stimulus. Changes in exercise selection, volume, intensity, novelty, and loading can influence the post-exercise response. With repeated exposure to a similar stimulus, soreness often decreases. Assess the response. Soreness is one piece of the picture. Consider function, performance, movement, range of motion, symptom trajectory, and tolerance of subsequent activity. Guide the next step. Use the patient's overall response, training history, and goals to determine whether the exercise dose should progress, stay the same, or be modified. The takeaway? DOMS isn't a badge of a successful workout. It's feedback. As physical therapists, our role is to put that feedback into context, not simply pursue or eliminate soreness. Advancing clinical skills through hands-on learning. Fort Worth Physical Therapy recently hosted an 8-hour CEU course in Fort Worth, TX, led by Leeda Rasoulian, PT, DPT, FAAOMPT, Board Certified Orthopaedic Clinical Specialist. Building on foundational mobilization and manipulation techniques, clinicians advanced their skills in: • Cervical, thoracic, and lumbar spinal mobilization and manipulation • Rib cage and SIJ examination and assessment • Intervention techniques, including costovertebral joint and SIJ manipulation Courses like this give clinicians the opportunity to sharpen hands-on skills, strengthen clinical decision-making, and immediately apply what they learn to patient care. From athletes on the field to the athletes of aging. Advanced Physical Therapy, a H2 Health company, recently hosted a continuing education course in Little Rock, Arkansas, led by Matt Gieringer, PT, DPT, MS, COMPT. Pain is real. But pain and tissue damage aren't always equivalent. Pain is a highly individual experience influenced by biological, psychological, and social factors. Tissue health matters, but so can sleep, stress, previous experiences, expectations, and the context in which pain occurs. For physical therapists, that means looking beyond a pain score. How is pain affecting function? What movements or activities have changed? What does the patient understand about their symptoms? What are they concerned about? And what do they want to get back to doing? Pain neuroscience education can be one part of that approach, particularly for some people with persistent musculoskeletal pain. Evidence suggests it may help improve pain-related beliefs and fear of movement, especially when integrated with active care rather than treated as a stand-alone solution. At H2 Health, the goal isn't to convince someone that their pain is “just” anything. It's to understand the person experiencing it, help make sense of what they're experiencing, and build an appropriate path toward function and meaningful activity. Better pain education shouldn't minimize pain. It should give patients more useful ways to understand and respond to it.
